Pre-Admission
In the Admissions Department, our mission is to provide tranquility to the patient so that he can be placed immediately in any of our clinical areas. The Admissions Department is responsible for coordinating all related steps to his hospitalization as it is referred to by a medical clinic or other hospitals. We have private rooms for the convenience of our patients and their families at an additional cost not covered by medical insurance.
Our offices are located on the first floor, next to emergency room.
Admission
Admittance to a hospital can be a complex procedure for some people. In Damas Hospital we strive to expedite the procedures required to facilitate the process of admission. In addition, we provide information that will be useful during your stay in our hospital.

On arrival, our patients must pass thru the Admissions Department, one of our employees will request their general data and the corresponding authorizations:

Medical Order, original, identified with full name of patient, date of admission and surgery (if applicable), signed by a doctor who is active in the Damas Hospital.

Informed consent (if applicable)

Insurance card

Photo ID

Referrals and/or permission of insurance(if applicable)

Evidence of address (water, electricity, telephone bill)

Social Security number

Deductibles and co-payments (if applicable)
Admission through Emergency Room
If a patient who visits emergency room is admitted to the Hospital, the staff in this area will provide necessary documents for admission. Once the patient is assigned to a room this is escorted by the staff of our institution.
For elective procedures pre-admission process
The pre-admission is a procedure that facilitates the admission process for elective surgeries and procedures. Our highly trained staff will guide our patients on the process to reserve the date of your admission. For the pre-admission process you need to bring the same documents required for your admission. The recommended time for the pre-admission is a minimum of five (5) working days prior to the date of the surgery or procedure. Accepted pre-surgery tests performed outside the hospital provided that they do not exceed 30 days at the time of the surgery or procedure.

Please have available the order of admission and insurance card at the time of your appointment.
Pre-certification and authorization
Your insurance may require approval before you receive certain services. It is important that you know the specific requirements of your insurance, so you can receive the maximum benefits assigned to the services that will be provided.
Before visiting Damas, please review your insurance policy benefits, or call your representative to verify your benefits and the requirements of the plan.
Information about payment
You are financially responsible for all services provided in the Hospital. These costs may include:

Payments can be made at any register window. Hospital Damas, Inc. accepts cash, personal checks, cash, Visa, American Express, MasterCard, ATH cards.
